I N F O R M A T I O N  P O L I C Y

All medical records and sensitive information is kept secure and confidential per The Health Insurance Portability and Accountability Act of 1996 (HIPAA). All forms containing such data including patient in-take forms are transferred either electronically via our encrypted HIPAA compliant online service portal, submitted in person, or by fax only. If you choose to submit in-take forms or other sensitive information to us electronically (Highly Recommended), you will be sent a secure link via email with easy-to-use instructions for submitting paperwork by way of our fully encrypted HIPAA compliant online service portal. Please DO NOT email referral forms to us, as public email is not a secure form of communication for an electronic health record.   

P A Y M E N T  P O L I C Y

Clients are expected to pay for consultations at or prior to appointment if paying without insurance.  Insurance co-pays are due at or prior to appointment. We accept all major credit card brands, cash, check, Health Savings Accounts, PayPal, and some cash transfer apps as valid forms of payment.  


We participate with all insurance companies listed on the Insurance Coverage page. Please check with your insurance company for your individual coverage. 


For out of network insurance carriers, a superbill can be provided upon request which you can submit to your insurance company.  Many insurance companies will cover the entire cost of visit.   

C A N C E L L A T I O N  P O L I C Y

Our office cancellation policy requires that clients must give at least 24 hour cancellation notice in advance of their appointment. If cancelling within 24 hours, but before the appointment window, you will be charged a cancellation fee of $30. A client who misses an appointment without any notice at all will be considered a no-show. No-shows will be charged the full out-of-pocket fee for their appointment, this is a maximum fee of $180. Fees will be waved only once in a calendar year. This fee will be billed to you directly and is not covered by your insurance. This balance must be paid prior to your next appointment. If you do not have a scheduled appointment, the balance is expected within in 60 days, and if not, may be subjected to collections. If late more than 15 minutes to the appointment your appointment will be cancelled, and you will be considered a no-show and will be held responsible for payment for the missed appointment. Three no-shows in one calendar year will result in a suspension of services.
